Asking for ‘Consanguinity’ in a Couple? — Rather Ask for ‘Endogamy’

In clinical genetics and genetic counselling, the term consanguinity — referring to marriage between blood relatives — is routinely asked about in patient histories. But in the Indian context, a more nuanced and culturally accurate term is often more appropriate: endogamy.

Consanguinity vs Endogamy

Consanguinity refers specifically to marriage between individuals who share a recent common ancestor — such as first cousins or uncle-niece marriages. It is associated with an increased risk of autosomal recessive genetic disorders in offspring, because both parents are more likely to carry the same recessive mutations inherited from a shared ancestor.

Endogamy refers to the practice of marrying within a specific social group, caste, community, or tribe — without necessarily being closely related by blood. In India, endogamy is the norm across most communities. While endogamy does not carry the same immediate genetic risk as consanguinity, over many generations it can lead to a “founder effect” — where certain genetic variants (including disease-causing mutations) become more prevalent within a community than in the general population.

Why This Matters for Genetic Counselling

When taking a family history, asking only about consanguinity may miss the broader genetic risk context in Indian patients. A couple from the same endogamous community — even if not closely related — may both be carriers of the same recessive condition that is prevalent in their community (e.g., thalassaemia in certain communities, G6PD deficiency, or specific metabolic disorders).

Understanding a patient’s community background and endogamous history allows genetic counsellors to:

  • Identify community-specific founder mutations that warrant targeted carrier screening

  • Provide more accurate recurrence risk estimates

  • Recommend appropriate genetic tests based on community-specific disease prevalence
