"Brain tumors are actually relatively rare," Dr. Ray reassures. "Primary brain tumors — tumors that originate in the brain — affect approximately 6–8 per 100,000 people annually. The vast majority of headaches — even severe, frequent headaches — are not caused by brain tumors. Tension headaches and migraines account for the overwhelming majority of headache presentations."
He emphasizes that the pattern of headache is more important than its severity. "A headache that has been present for years and follows a consistent pattern is very unlikely to be caused by a brain tumor. It's the new headache — particularly one with specific warning features — that warrants urgent evaluation."
Warning Signs That Require Urgent Evaluation
Dr. Ray outlines the "red flag" features that should prompt immediate medical attention:
- Thunderclap headache: A headache that reaches maximum intensity within seconds — "the worst headache of my life." This pattern is a medical emergency and may indicate subarachnoid haemorrhage.
- New headache in someone over 50: New-onset headache in older adults warrants evaluation to exclude secondary causes.
- Headache with neurological symptoms: Weakness, numbness, vision changes, speech difficulties, or confusion accompanying a headache require urgent evaluation.
- Headache that wakes you from sleep: Particularly if it's worse in the morning and associated with nausea and vomiting — this pattern can indicate raised intracranial pressure.
- Headache with fever and neck stiffness: This combination suggests meningitis — a medical emergency.
- Progressive worsening: A headache that is steadily worsening over days to weeks warrants evaluation.
- Headache in someone with cancer or HIV: New headache in these contexts requires urgent evaluation to exclude brain metastases or opportunistic infection.
The Genetics of Brain Tumors
"Most brain tumors are sporadic — they arise from random mutations without a clear hereditary cause," Dr. Ray explains. "However, a small proportion — approximately 5–10% — occur in the context of hereditary syndromes." These include Neurofibromatosis type 1 (NF1 gene), Neurofibromatosis type 2 (NF2 gene), Von Hippel-Lindau disease (VHL gene), Li-Fraumeni syndrome (TP53 gene), and Cowden syndrome (PTEN gene).
"For patients with a family history of brain tumors or known hereditary syndromes, genetic counselling and appropriate surveillance are important," he advises. "And for all brain tumor patients, tumor molecular profiling is now standard of care — identifying the specific genetic alterations driving the tumor guides treatment selection and predicts prognosis."
