Endometriosis: The Pain Is Real
Endometriosis affects approximately 1 in 10 women of reproductive age worldwide — an estimated 190 million women globally, and over 25 million in India. Despite its prevalence, endometriosis is chronically underdiagnosed, with an average diagnostic delay of 7–10 years. The pain is real, the condition is serious, and awareness is urgently needed.
What Is Endometriosis?
Endometriosis is a chronic inflammatory condition in which tissue similar to the lining of the uterus (the endometrium) grows outside the uterus — on the ovaries, fallopian tubes, the outer surface of the uterus, the bowel, bladder, or other pelvic organs. Like the uterine lining, this tissue responds to hormonal changes during the menstrual cycle — thickening, breaking down, and bleeding. But unlike menstrual blood, this blood has nowhere to go, causing inflammation, scarring, and adhesions.
Symptoms
Severe, debilitating menstrual pain (dysmenorrhoea) — far beyond typical period cramps
Chronic pelvic pain, including between periods
Pain during or after sexual intercourse (dyspareunia)
Pain with bowel movements or urination, especially during menstruation
Heavy menstrual bleeding or bleeding between periods
Infertility — endometriosis is found in 30–50% of women with infertility
Fatigue, bloating, nausea, and digestive symptoms
The Genetic Angle
Endometriosis has a strong hereditary component — women with a first-degree relative (mother or sister) with endometriosis have a 7–10 times higher risk of developing the condition. Genome-wide association studies (GWAS) have identified multiple genetic loci associated with endometriosis risk, including variants in genes involved in oestrogen signalling, immune function, and tissue remodelling. Diagnosis and Treatment
Definitive diagnosis requires laparoscopy (keyhole surgery). Treatment options include hormonal therapies (contraceptive pill, progestins, GnRH agonists), pain management, and surgery to remove endometrial lesions. Fertility treatments may be needed for women with endometriosis-related infertility.
